Surface RT and ustream.tv

Was trying to set up Surface RT to broadcast using ustream, but it was unable to detect the audio source or video source. Both of these work fine using other apps such as Skype. Ustream is on the list of approved flash sites (however I did not see the usual flash player settings popup window asking for camera and mic access).

Any ideas on how to get it to work would be appreciated.
