Help diagnosing a dGPU issue

Well when I turned on my SB last night and checked for updates a Nvidia driver update was out there. Did you install the new driver?

I just ran a check for updates and nothing. Can you tell me what the driver version is from the device manager?
Update 1: Got the SB to download the 364.72 driver (per the 5/19 SB update) - did not fix the issue
